Entering a new case

| The
first time you start StatTracker, you are presented with a mostly
blank screen that will become the list view of your cases. This
is analogous to the built in address book application, which
has a name and one phone number on each line. When you tap on
a name, you are taken to another screen with the details of
that case. But first you need to enter a case to see a name
in the list view. To do so, tap the New button at the bottom
left. |

| You'll
then be presented with a blank "Enter Case" screen.
Enter the patient's name, medical record number, and procedure. |

| The
first field, the date, is filled in auotmatically with today's
date. To enter another date, simply tap on the date numbers
inside the dotted box to be taken to this date selection screen: |

| Next
choose all that apply from the lists of checkboxes that popup
when tapping one of the four large buttons. These are the categories
specified by the American Council for Graduate Medical Education
for end of academic year reporting. |

| Add
any notes that you would like to keep for the case by tapping
on the "Notes..." button. This field will scroll automatically
up to a maximum of 300 characters. Hit the "Done"
button when you are finished. |

| That's
it! Now simply tap on "Done" and you will be taken
back to the main screen with the list of cases. |
